Key Responsibilities: The PHN Level 5 is responsible for delivering preventive, promotive, and curative health services with a focus on maternal, child, and reproductive health, as well as community health programs. The duties are categorized as follows:
- Program Planning and Resource Management:
- Assist in preparing plans for the procurement, distribution, and storage of medicines, equipment, and other health resources.
- Ensure effective management of resources (human, material, and equipment) to support health programs.
- Administrative and Management Tasks:
- Perform tasks as per the job description and delegated responsibilities.
- Report progress and challenges to the relevant supervisor regularly.
- Coordinate with the office head to arrange necessary resources for program implementation.
- Ensure smooth and quality delivery of health services as per guidelines.
- Health Service Delivery:
- Safe Motherhood and Family Planning:
- Provide counseling and services for regular and emergency family planning.
- Facilitate access to sterilization services through camps or health facilities.
- Ensure quality implementation of safe motherhood and family planning programs at hospitals, primary health centers, health posts, and birthing centers.
- Arrange maternal and newborn care services, including essential supplies, medicines, and equipment.
- Monitor and maintain the quality of safe motherhood services.
- Safe Abortion Services:
- Coordinate with health facilities to ensure continuous availability of safe abortion services.
- Support the management and implementation of safe abortion programs.
- STI and HIV/AIDS Prevention:
- Identify target groups and arrange Voluntary Counseling and Testing (VCT) and Prevention of Mother-to-Child Transmission (PMTCT) services.
- Promote and distribute condoms to prevent STIs and HIV/AIDS.
- Coordinate with government and non-government organizations to activate STI/HIV coordination committees.
- Adolescent Reproductive Health:
- Establish and support adolescent-friendly health services and information centers in schools.
- Provide counseling and referral services for adolescent reproductive health issues.
- Infertility Prevention and Treatment:
- Arrange counseling and referral services for women with reproductive health issues, including infertility.
- Immunization:
- Ensure regular immunization services through health facilities or mobile clinics.
- Support the implementation of iron-folic acid supplementation programs for schoolchildren and management of severe malnutrition cases.
- Safe Motherhood and Family Planning:
- Female Community Health Volunteer (FCHV) Program:
- Develop work plans for FCHVs and ensure the provision of registers, forms, guidelines, and identification cards.
- Arrange observation visits, awards, and incentives for FCHVs.
- Facilitate basic and refresher training for FCHVs to enhance their service delivery.
- Coordination and Collaboration:
- Coordinate with health offices, hospitals, local governments, primary health centers, and birthing centers to ensure regular health service delivery.
- Collaborate with private and non-governmental organizations for effective health program implementation.
- Act as a coordinator for reproductive health coordination committee meetings.
- Provide feedback on program progress and challenges to higher authorities.
- Monitoring, Supervision, and Evaluation:
- Supervise reproductive health activities, providing guidance and support to staff.
- Monitor services like safe motherhood, safe abortion, and adolescent health, addressing issues such as quality and coverage.

- Training and Capacity Building:
- Arrange training for staff on safe motherhood, family planning, safe abortion, and adolescent reproductive health.
- Support post-training service delivery to ensure quality and continuity.
- Participate in health-related research and studies to improve service delivery.
- Research and Development:
- Engage in health-related research and studies conducted by relevant offices or agencies.
